Our physicians and Nurse Practitioners are medical specialists in hospitals and long-term care facilities. We start with initial admission into the facility, then we diagnose and treat illnesses of all kinds. If your treatment requires it, we consult with other specialists such as cardiologists or nephrologists to better treat your illness. While you are in the facility, we will coordinate your care by overseeing the team of specialist.
Our goal is to have you in and out the facility as soon and as healthy as possible.

When you are admitted as an inpatient, our team will work with the patient and the PCP to create with treatment options to meet yours goals.
Seamless transition from the hospital (inpatient care) back to Patient Primary Care (PCP) Physician
At MD Healthcare Direct, we believe in continuity of care. You and your PCP are the key to that. So, when you are discharged from the hospital or long-term care, we direct you back to your PCP for follow up care.
